Granular, informed, time-bound and withdrawable. Each of those words does work.
Structure

Four properties
Granular
A parent may consent to screening but decline the card, or decline a specific investigation, without declining the programme.
Informed
Presented before collection, in a language the person reads, at a length they can actually read in a waiting room.
Time-bound
A sharing permission has an end. It does not run indefinitely because nobody revoked it.
Withdrawable
At any time, with a stated consequence for the existing record.
What consent does not gate

The emergency view
Blood group, allergies, conditions, medications and emergency contact are readable from the card without a consent step, because in an emergency there is nobody to ask. This is a deliberate trade and it is the reason the emergency view is kept as small as it is.
