Effective Practice Incentive Community
Healthcare quality improvement through an Integrated and Interoperable Digital Health Profile. The annual clinical picture, built on top of the RISE emergency baseline. Optional, and advised annually.
Three upgrades over RISE
A card that carries imaging
32 GB SD-chip enabled, FHIR compliant, capable of storing medical records including CT and MRI studies. The imaging travels with the patient.
A fuller laboratory panel
Blood grouping and complete blood picture, plus thyroid profile and lipid profile — the panels that catch what a school-age population actually presents with.
Four disciplines, not one
Paediatrician or general physician with 4Ds screening, plus dental, ophthalmology and ENT screening and consultation.
Growth is the point
An annual health profile is a tool for monitoring a child's growth and development and for identifying potential health issues that need further attention. A single measurement tells you very little; a series tells you a great deal.
A comprehensive profile should include medical history, current health status, family history of illness, and any medications or supplements being taken. Repeated each year against the same identity, it becomes the child's actual health history.

Who should be in the cohort
Run EPIC this academic year.
Best scheduled early in the year, so referrals have time to close.
