What each side commits to
A short memorandum, not a long contract. Both parties should be able to read it in ten minutes.
What the Foundation commits to
Qualified clinicians
Every screening station staffed by a registered clinician in the relevant discipline.
Named dates
Camp dates agreed in advance and honoured, or rescheduled with notice and at our cost.
Data handling
Identifiable data held only as consented, in India, and never shared with the school beyond what the school needs.
Referral support
Counselling for every flagged child and a named referral destination.
Reporting
An annual disease statistical report within a stated period after the final camp day.
Price
The published schedule, with no variation introduced after signature.
What we need from you
Space and power
Two private consultation spaces, a waiting area, power and a registration table.
An accurate roll
The list of students and staff, reconciled before the first camp day.
Consent distribution
Circulate the consent letter and collect responses, in the languages your families read.
A named owner
One person empowered to make decisions on the day.
Follow-up cooperation
Tell us whether referred children attended, so the loop can close.
Payment terms
As set out in the schedule, or confirmation of the CSR partner funding it.
The clauses worth reading twice
The school is not the data holder
Individual clinical records belong to the child and the family, held under the child's ABHA identity. The school receives aggregate reporting only.
Consent can be partial
A parent may consent to screening but decline card issue, or decline a specific investigation. The memorandum requires the school to pass that through.
Withdrawal is possible
A family may withdraw consent afterwards. The memorandum sets out what happens to the record when they do.
